Preparing for Adulthood - Cornwall Council's Transition Team
Services and organisations that have the Local Offer flash have filled out inclusion information, meaning that they meet the needs of people with Special Educational Needs and Disabilities (SEND)
Our transition workers help children and their families or carers make the transition to adult care and support.
Your transition worker will try to see you when you are 16 years old to help you think about the support you might need when you are an adult.

Eligibility for Adult Social Care

- How we make our service inclusive
The Transition Team Members remit is to support young disabled people in preparing for adulthood. They have a wide range of experience and are skilled in various communication methods

Wide range of communication methods including makaton.
- How our service supports providers / schools / colleges
The Transition Team work in partnership with schools, colleges and providers as part of multi-disciplinary team in order to support young disabled people in preparing for adulthood.
- Support at key transition points
The Transition Team support young disabled people, primarliy between the ages of 14- 18, to prepare for adulthood. Their remit is to facilitate a smooth transition from Children's to Adult Services.
