If you or someone you know is looking for a place to live or a short break away with support on hand, Shared Lives South West may be able to help.

We specialise in the provision of family-based care and offer a range of long term, short break and day services with our approved carers.  We are able to support people with a learning disability, mental health needs, older people, people with early stage dementia and parents with a learning disability.  Some of our carers are also able to support people with physical disabilities. Our service is available to adults, although we can support young people aged 16+ in transition to adult care. Please contact us to discuss your needs and/or have a look at our website.

How we make our service inclusive

We provide short breaks/respite for vulnerable adults and this can include those coming up to age 18 and in the transition process.  However, as our carers deliver the service in their own family homes our ability to match the needs of the young person with the skills and experience of our carers can often also depend on the availability of carers with accessible homes (e.g. ground floor living accommodation/wet room etc).

We can also provide long term places in family homes for over 18s where the person concerned wishes to live independently of their family or family members are finding it difficult to cope for a variety of reasons.  Again availability of places with our carers cannot be guaranteed.

Staff SEND experience

Our team of Shared Lives co-ordinators match people referred to us to carers who have the abilities, skills and experience to support them.  Our co-ordinators themselves have a varied range of skills and experience from the social care world - for example working with young people on the Autistic spectrum or who have learning disabilities.

How our service supports providers / schools / colleges

We are happy to come along to transition events or talk to school staff individually about what we can offer to young people with a range of special needs who are coming up to age 18 and beyond.

Support at key transition points

We can provide a caring option, which may or may not be appropriate for the young person transitioning to adulthood depending on the level of need.  We are happy to talk to families about what we can offer.

Resolving disagreements, mediation and making complaints

We have a Complaints Policy which includes details of how complaints can be made and how they are dealt with.  This includes both informal and formal complaints.  We also have a Customer Service policy which sets out standards for how we deal with  anyone accessing our service.
