Children in Care Team (CIC), Children’s Community Therapy Services (CCTS) and Community Paediatrics work in partnership within the department of Community Child Health at the Royal Cornwall Hospital NHS Trust.
The Service is community focussed and therefore will be delivered from appropriate locations and within suitable settings in order to ensure an effective service for children and young people, close to where they live. This may include the service user’s home or place of residence or nursery or school as well as local clinic settings and the hospital.

Who is Eligible?
Children Looked After and Care Leavers
The service will accept referrals from children and young people Looked After or Care Leavers, their carers or Social Workers or other Health Professionals.
No referral form required but we require either a telephone conversation or a formal referral letter summarising the current difficulties and any relevant history.

- How we make our service inclusive
• The Children in Care Health Team work in partnership with colleagues from Cornwall Council, other placing Local Authorities and Independent Care providers to ensure that the health needs of children in care and care leavers are met. • The Team offers training on a number of aspects of children in care health care and this is usually provided in combination with appropriate partners and is suitable for those with the responsibility for the care of looked after children. • The Team works in close partnership with Community Paediatricians, Community Therapy Teams as well as Health Visitors, School Nurses, Social Workers and Specialist Educational Advisors. • Multi professional working with other health providers and Agencies to enable local delivery of appropriate care pathways for children who are vulnerable and or with complex health needs. • The Children in Care Health Team is involved in the statutory assessment of health needs and the planning and delivery of healthcare and health promotion to children in care and care leavers.
- Staff SEND experience
The Children in Care Health Team works with children and young people who present with a range of complex needs and disabilities. All staff are qualified Specialist Public Health Nurses and have a range of experience working with children and young people with complex needs and disabilities. All staff have access to additional training and regular updating.
- Support at key transition points
The service will support young people and parents/carers in developing their capacity to reduce the health consequences of long term vulnerability and chronic medical conditions. This will include the appropriate provision of written materials and signposting to other support services. We are also able to provide advice towards the annual Education Health and Care plans including those that target transition.

The Children In Care Team actively seeks your views and experience of the service. Any concerns should be raised with your child’s CIC nurse, or if this is not appropriate you should contact the Lead Nurse for the CIC Team.
