          Cornwall and Isles of Scilly (IOS) Sustainability and Transformation Plan (STP)

          The STP is a plan for how health and social care in will work together over the next 5 years.

          The plan aims to:

          • improve health and wellbeing of local people
          • improve the quality of local health and care services
          • make the services good value for money so that we can keep running them for a long time

          The plan has been written by health and social care services, voluntary and private organisations and the people who use the services.

          Local Transformation Plan (LTP) for children and young people’s emotional mental health and wellbeing

          The Cornwall and IOS's Local Transformation Plan is called Turning the Tide.  This plan updates and replaces the Joint CAMHS Strategy Implementation Plan issued in October 2015.  The plan aims to improve the emotional wellbeing and mental health of all our children and young people, to build their resilience and to ensure that those who experience emotional wellbeing and mental health problems get the right help at the right time. 

          Transforming Care Partnership Board (TCPB)

          Cornwall Council, the Council of the Isles of Scilly and NHS Kernow Clinical Commissioning Group have formed a Transforming Care Partnership Board.

          The Transforming Care Partnership (TCP) have produced a easy read version of the Transforming Care Plan

          This plan is for children, young people and adults with a learning disability and/or autism who have challenging behaviour. The plan aims to ensure that:

          • services have training on understanding people’s needs and person centred planning
          • ensure people on the autistic spectrum have their needs met
          • children and young people across Cornwall and the IOS have access to the same good care and that transition from children’s to adults services is good
          • people have a choice of where they live and that their housing meets their needs
          • people will have access to person centred planning and personal budgets
          • people at risk of going into hospital are given support and less inpatient beds are used
